A special form of methionine, S-adenosyllethionine (S-adenosyl methionine) (SAM-e), is an important physiological factor formed in the body by the synthesis of the necessary amino acid methionine with adenosyltriphosphate (ATP). Due to the fact that it has so many actions in different parts of the body, SAMe can have many health benefits that seem unrelated, but in reality it all depends on SAMe. SAMe is a popular dietary supplement for the treatment of osteoarthritis and depression but, as we will see, it may be useful for those suffering from rheumatoid arthritis.

SAMe and liver
SAMe can stimulate liver regeneration and restore or reverse cirrhosis-induced liver damage. In the liver, SAM-e performs two basic functions: methylation and formation of the most essential substance - the amino acid glutathione, which is both an antioxidant and a detoxifying agent. SAMe acts as a precursor to the production of glutathione (1). SAM-e is converted to cysteine ​​in the body, and N-acetyl-L-cysteine ​​increases glutathione in the body.
Clinical studies show that increasing levels of SAM-e produce benefits in many cases. It has been used to treat various liver conditions, such as hepatitis, cirrhosis, cholestasis and obstructive jaundice (2).
Cirrhosis patients often have difficulty converting methionine to SAM-e. Human studies indicate that when SAM-e is administered to these patients, bile function is restored and bile salts are increased. Other studies indicate that SAM-e increases glutathione levels and restores detoxification processes in patients with hepatic impairment (3-4).
SAM-e can provide significant benefit to patients suffering from gall bladder disease. Elevated levels of female hormone estrogen can increase the amount of cholesterol secreted in the gallbladder, putting women - especially those who have multiple pregnancies and those using estrogenic drugs - at a higher risk of gallstones. In a study of six healthy women who received oral contraceptive estrogens, daily 600 mg SAM-e treatment reduced cholesterol by nearly one-third, suggesting that SAM-e may prevent the formation of gallstones in women with increased levels of estrogen (5).

SAMe and depression

Certain psychiatric, neurological and metabolic problems are associated with lower SAM-e levels in cerebrospinal fluid.
SAM-e has been shown to be effective in relieving all forms of depression outside bipolar depression. It is particularly beneficial in cases of depression for which there is no obvious cause. SAM-e is thought to exert its antidepressant effect by increasing the output of neurotransmitters that play a key role in mood, behavior and emotions. SAM-e affects the metabolism of brain neurotransmitters dopamine and serotonin, both of which contribute to mood regulation (6). It penetrates the almost impenetrable barrier between the blood and the brain, the blood-brain barrier, a barrier that prevents many drugs from acting in the central nervous system (7). SAM-e levels are lower in the spinal fluid of people with depression (8), and elevated blood levels of SAM-e have been found to cause improvements in depressive symptoms (9).
Too many studies have shown that SAM-e is at least as effective in treating depression as pharmaceutical antidepressants, and sometimes more effective. In addition, these studies indicate that SAMe has few side effects, acted within a few days and was well tolerated without side effects (10-12).
Studies have shown that SAM-e improves patients with fibromyalgia and Parkinson's disease who suffer from depression (13). SAM-e increases serotonin levels, a neurotransmitter that relieves pain and depression - two very common symptoms of fibromyalgia.
46% of patients with major depression show either a partial response or no response to antidepressant therapy, underlining the need for strategies to improve the effectiveness of depression therapy. In 2004, the researchers studied the complementary effects of SAM-e and modern antidepressant drugs (14). 30 patients who had inadequate responses to SSRIs or venlafaxine received 800-1600 mg daily SAM-e for six weeks. 50% of these resistant patients improved with the addition of SAM-e in their treatment, and 43% experienced remission of the symptoms. The authors concluded that increasing antidepressant therapy with SAM-e may be effective in relieving depression-resistant treatment.

SAMe and arthritis

The lack of SAMe in the joints tissues, just like the lack of glucosamine, leads to its loss as a cartilage-like gel and its ability to absorb vibrations. From this the researchers began to understand that SAMe may be useful in treating osteoarthritis. In a double-blind study, SAMe supplements increased cartilage formation, as determined by MRI, in 14 patients with osteoarthritis in the hands (15).
In a study published in the British Journal of Rheumatology, researchers have shown that SAMe has reversed the effects of injury to the joints caused by tumor necrosis factor (known in the international literature with the original TNF) when SAMe was added to the cells at the same time with TNF (16).
Almost all the studies on arthritis that have been done with SAMe are about osteoarthritis. In the laboratory tube, SAMe increases the number of chondrocytes (cartilage cells) and proteoglycans (proteins). This indicates that SAMe therapy can reverse the deeper process of osteoarthritis, causing cartilage growth (17-18).
The other major component of the joint is the synovial fluid, which acts as a lubricant. Inflammatory cytokine TNF-alpha (tumor necrosis factor-alpha) has been found in the synovial fluid of people with rheumatoid arthritis and plays a role in the destruction of bone and cartilage (19). Until recently, scientists were unaware of the effects of SAMe on synovial fluid. It was discovered that SAMe reverses the damaging effects of TNF-alpha.
In 1987 the American Journal of Medicine published a series of articles on the use of SAMe for the treatment of osteoarthritis. Studies have confirmed that SAMe works just as well as most popular antiarthritic therapies. The series was published under the title "Osteoarthritis: Clinical picture, pathogenesis and management by studies in a new therapeutic agent, S-adenosylmethione" (The exact title in English was: "Osteoarthritis: The Clinical Picture, Pathogenesis, and Management with Studies on and the New Therapeutic Agent, S-Adenosylmethione ").
Many years earlier, studies in Italy have shown that they have shown the benefits of SAMe. One of these studies included approximately 22,000 patients. This clinical trial lasted two months. Participants were not allowed to take painkillers or other anti-arthritic therapies during the study. Doctors found that patients taking SAMe had improved steadily from scratch. At the end of the study, about 80% of the subjects receiving SAMe reported improvement. 70% of those with the most severe knee pain were significantly improved. The side effects were minimal and only 2.3% of the group stopped taking it because it had no positive results.
In four double-blind studies, SAMe was compared with either placebo or non-steroidal anti-inflammatory drugs. SAMe was generally better tolerated by non-steroidal anti-inflammatory drugs (20-23).

SAM-e PLUS contains ingredients such as tryptophan, griffonia-simplicifolia, herbs and B-complex vitamins that increase the anti-depressant effect of SAM-e and help the central nervous system.
