Selenium is a trace element nutritionally essential for humans, playing a critical role in reproduction, thyroid hormone metabolism, DNA synthesis, immunocompetence and protection from oxidative stress and infection.

Selenium intake as well as its plasma levels vary depending on population, the selenium soil content, food quality and intake. Foodstuffs rich in selenium are Brazil nuts, organ meats, poultry, seafood, grains and dairy products.

Selenium actions are mediated through its incorporation in selenoproteins. The most important are the strong antioxidant glutathione peroxidase (GPx), protecting thyroid cells from the excessive H2Oproduction during thyroid hormone synthesis, and the deiodinases acting by deiodination (removal of an iodine atom) from thyroxine molecule thus catalyzing the formation of the hormonally active triiodothyronine.

Thyroid gland is the organ with the highest selenium content in the body. Recent epidemiological studies have shown that low plasma selenium levels are associated with higher incidence of goiter, thyroid nodules, and autoimmune thyroid disease such as Grave’s disease and Hashimoto’ thyroiditis. Consequently, selenium deficiency constitutes a risk factor for morphological and functional disorders of the thyroid gland.

Selenium administration to patients with autoimmune thyroiditis (Hashimoto) with or without thyroxine replacement affects the underlying autoimmune status as shown by the time dependent gradual fall of thyroid antibodies and the associated improvement of gland echogenicity and patient’s quality of life.

The additional selenium administration in euthyroid patients with Grave’s disease and mild ophthalmopathy decreases the clinical activity score and slows its progression with concomitant improvement of quality of life. These findings have led the European group on Orbitopathy to incorporate selenium administration into the guidelines for treating mild Grave’s Orbitopathy.

Selenium administration in patients with Hashimoto’s thyroiditis during pregnancy results in improvement of gland echogenicity, fall of thyroid antibodies, delay in the progression of the disease and reduction of the incidence of post-partum thyroiditis and permanent hypothyroidism.  

Daily selenium uptake in the Greek population is lower than the optimum as indicated by specific studies taken place in different places in the Country.
